You can only join as a single if your spouse has their own plan. All family members eligible for the group plan must be added unless they have alternative group benefits.

Please contact us within 31 days of any change to avoid medical (ie. Add a baby to the plan, add a spouse, etc).

Every dentist should accept your card. If they don’t please have them call the number on the card. Often the issue can be solved with the call. If you are forced to pay you will typically be refunded by your insurance company within 24 hours.

This could be caused by multiple reasons (spelling your name, date of birth) and can be solved with a call as listed on the card.

Most plans insist on generic only. The plan will only cover the cost of generic. Some plans have the ability for the doctor to insist on brand name drugs and therefore "will be covered by your plan. All plans, if necessary, include a form for the doctor to complete to allow brand name drugs and to be covered by the plan."

Your plan does not work once you leave the country, however your group travel plan kicks in for emergency medical coverage only.

You will need to fill in your co-ordination of benefits so that you can be covered by both plans or once member can waive benefits.

You can pay for optional life, AD&D or critical illness added onto your group plan or contact us for more individual options.

Once your child turns 21 they can only stay on the group plan if they are a full-time student. Every year from age 21-26 you will have to provide proof of tuition in order to keep them active on the plan.

Benefits continue uninterrupted; however you are no longer eligible for Long Term Disability and Life insurance is 50% of the value prior to age 65 and is zero at age 70.

A beneficiary form can always be filled out to update beneficiaries, please contact us if a form is needed.

All employees can be offered different plans as long as they are divided up into different classes.
